Easton-Hogg Psychiatry & Healthcare is committed to protecting health information. This page describes how protected health information may be used and disclosed and summarizes the rights available to you.
Your rights
- Request an electronic or paper copy of your medical record.
- Ask to correct information you believe is inaccurate or incomplete.
- Request confidential communication by an alternative method or location.
- Ask the practice to limit certain uses or disclosures.
- Request a list of certain disclosures of your information.
- Receive a paper copy of the privacy notice.
- Choose someone with legal authority to act for you.
- File a complaint without retaliation.
Your choices
In certain situations, you may tell the practice how you want information shared, including communication with family members or others involved in your care. Written authorization is generally required for uses not otherwise permitted by law, including most uses of psychotherapy notes, marketing, or sale of information.
Uses and disclosures
The practice may use or share information as permitted by law for treatment, healthcare operations, billing and payment activities, public-health and safety needs, legal requirements, workers’ compensation, and other government or legal requests. Substance-use-disorder records may have additional protections under federal law.
Our responsibilities
The practice is required to maintain the privacy and security of protected health information, notify affected individuals when a reportable breach occurs, follow the current privacy notice, and provide a copy upon request.
Questions or complaints
Contact the office at (859) 756-3630 for privacy questions, records requests, or complaints. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights through its HIPAA complaint process. The practice will not retaliate against you for filing a complaint.
